一、服务对象

全县开展送教上门的服务对象为确实不能到普通学校随班就读、但又具有一定接受教育能力的义务教育阶段6—14周岁的残疾儿童少年。主要包括：重度残疾儿童少年；多重残疾儿童少年；因病休学的部分残疾儿童少年；因入学较晚，超出学龄，未完成义务教育的部分残疾儿童少年；因其他原因未完成义务教育学习任务的部分残疾儿童少年。

二、服务原则 

（一）免费教育原则。送教上门服务属残疾儿童少年九年免费教育范畴，接受服务的家庭及个人不用缴纳任何费用。

（二）家庭自愿、协商一致原则。充分尊重家庭意愿，在自愿的基础上，送教上门服务的学校与重度残疾儿童少年家庭合作开展，在平等深入协商、就各方需承担的工作责任等事项取得一致意见后，签订送教上门服务协议，按协议开展服务。

（三）定期送教原则。提供送教上门服务的学校，按每个服务对象的具体情况与家长协商确定学习周期，可按周、月、季进行安排。按照一般每月不少于2次，每次不少于2个课时，每学年不少于40个课时的时间，制定服务工作日程表，开展送教上门报务工作。

三、送教内容

送教内容主要包括残疾儿童身体康复、心理康复、潜能开发、认知能力、语言能力、运动协调能力、生活自理能力和社会适应能力的培养训练等。负责送教上门工作的教师应根据每个重度残疾少年儿童的实际情况，选择合适的教学内容和教学方法。原则上，教学内容以生活语文、生活数学、英语字母、生活常识、运动康复为主，不统一教材，可参照幼儿园、小学阶段课本自行选择和编写教学内容；多采用直观式、游戏式教学方法，循序渐进，努力提高重度残疾少年儿童的认知能力和适应生活、适应社会的能力；送教上门服务对象较多的学校，也可以采用个别送教与集中授课相结合的方式进行；加强康复课程的开展，对有需要的儿童少年可联合其他职能部门，配发一定数量康复器材；对于文化和技能都没有接受能力的个体，主要以康复服务与爱心扶助为主。

四、师资保障

由学校选派责任心强、热爱残疾学生，思想、业务水平较高且具有丰富实践经验的教师，开展“送教上门”工作。送教教师工作必须认真负责，对服务对象负责，对送教教师的集体形象负责；尊重服务对象，爱护服务对象，为服务对象的身心健康成长尽心尽责。要针对“送教上门”服务对象的生理、心理特点和残疾类别，制定切实可行的教育教学计划，多形式地选择“送教上门”服务方式，努力提高送教质量。

五、工作要求

（一）加强组织领导。提高残疾儿童义务教育普及水平，是《第二期特殊教育提升计划（2017-2020年）》一项重要内容，是推进教育公平、实现教育现代化的重要任务，是增进残疾人家庭福祉的重要举措。各相关学校要高度重视此项工作，成立专门的工作小组，主要领导亲自抓，分管领导具体抓，骨干老师带头干。按照“一人一案”的原则，制订“送教上门”工作方案。

（二）加强过程管理。各相关学校要为本校“送教上门”学生建立学籍，确保学籍到位。要建立“送教上门”工作档案，档案资料主要包括学生基本情况、“送教上门”相关图片、教学资料等。每学年初、学年末各相关学校要举行送教教师工作座谈，研究计划，总结工作。

（三）加强督导检查。县教科体局将不定期对送教上门工作进行动态考核督查，对“送教上门”学生学籍建立不到位、送教工作落实不到位、家长反映比较大的，将按照相关规定扣减目标绩效分值。
