高血压是全球人类最常见的慢性病。心脑血管病的发生和死亡一半以上与高血压有关，控制高血压是防治心脑血管病的切入点。

高血压的定义：在未用抗高血压药的情况下，非同日三次测量，收缩压≥140mmhg和/或舒张压≥90mmhg，可诊断为高血压。患者有高血压史，目前正在服用抗高血压药，血压虽低于140/90mmhg，也应诊断为高血压。

（一）老年人：老年（＞65岁），降压药务必从小剂量开始，根据耐受性逐步降压，应测量用药前后坐立位血压；尤其对体质较弱者更应谨慎。老年人有较多危险因素、靶器官损害、合并心血管病、糖尿病等情况也较多，常需多药合用。

（二）冠心病：稳定性心绞痛时首选β受体阻滞剂或长效钙拮抗剂及长效acei；急性冠状动脉综合征时选用β受体阻滞剂或acei；心肌梗死后病人用acei、β受体阻滞剂和醛固酮拮抗剂。
