全面贯彻党和国家的教育方针,规范办学行为,深化教育改革,提高教育教学质量。以市教育局“规范办学通知”和“禁止在职教师有偿办班补课的通知”,加强师德师风建设,坚决整治教师有偿办班补课之风,切实减轻学生的负担,为全面实施素质教育创造良好的条件。

1、组织领导,强化责任。为切实加强本次活动的组织领导,学校成立开展治理教师有偿补课专项工作领导小组。

3、宣传教育

接到教育局通知后,我校立即成立领导小组,制定实施方案,并召开全体教职工大会,组织教师认真学习《教育部等五部门关于20xx年规范教育收费治理教育乱收费工作的实施意见》和省、市教育行政部门制定的关于严禁在职教师乱办班、乱补课的文件、通知和各项规章制度,公布了实施方案,最后,和每一位教师签订了禁止在职教师有偿办班补课的承诺书。

学校将组织有关人员采取定期不定期的方式,对我校教师有偿补课情况进行明察暗访、督导检查,并将督查情况及时进行反馈和通报。向社会和家长公布举报电话和校级主要领导的手机号码,接受群众监督。对群众反映和举报的每一个个案都要进行认真调查核实,做到发现一起,查处一起,绝不姑息迁就。

3、工作总结

八月中旬,学校将按上级主管部门的要求,对严禁教师有偿补课专项工作进行认真总结,正确分析师德师风的现状,深刻剖析存在的问题和成因,不断吸取经验教训,提出今后的工作对策,切实有效地把治理教师有偿补课活动深入开展下去。

1、加强教育

我校按照市县教育局的部署,结合我校实际,制定出台《关于教师有偿办班补课的处理意见》。采取有力措施,加大师德师风建设力度:一是认真学习《中小学教师职业道德规范》,从思想上明确作为一名教师应遵循的职业道德规范。二是集中表彰一批优秀教师、优秀班主任、优秀辅导员等;三是召开班主任、辅导员经验交流会。加大宣传师德先进典型力度,树立教师正确职业道德观念。

2、完善制度建设

规范教师教学职责,加强对教学工作的考核和监控,要求教师备好每一份教案、上好每一节课。继续签订师德师风建设责任状与承诺书。加强对教师师德师风情况的考核,把教师的思想政治素质、职业道德水平和教书育人能力,作为教师年度考核、职务评聘和评先评优等的重要依据。完善师德师风建设监督机制,公开师德师风建设举报电话,设立师德师风建设举报箱,建立学校、教师、家长、学生“四位一体”的师德建设监督网络。

3、严格规范学校办学行为

学校严禁教师在校内外办班收费补课,不将校舍和场地出租、出借给社会培训机构或校外人员开展针对中小学课程的有偿补习活动。学校不搞任何形式的有偿补课活动。

4、做好教师绩效考核工作

开展教职工绩效考核是加强学校管理、规范教师行为、推进依法治教的重要基础。是提高教师队伍整体素质、促进教师队伍科学发展的关键环节。

教师绩效主要考核教师履《教育法》、《义务教育法》、《教师法》等法律法规规定的教师法定职责,履行学校的的岗位职责和完成工作任务的实绩。包括师德和教育教学、从事班主任工作等方面的实绩。

5、严禁教师违规办班有偿补课

不准教师以任何名义违规办班有偿补课、强化训练或参与其中有偿授课;严禁教师违规从事有偿托管;未经学校批准,教师不准参加社会举办的各种以升学为目的的培训班、补习班等兼职补课活动。

6、严肃查处违规有偿补课行为

按照党风廉政建设责任制和教育部《关于贯彻〈义务教育法〉进一步规范义务教育办学行为的若干意见》,省监察厅、省教育厅《关于违反教育收费管理政策法规行为纪律处分的暂行规定》追究有关人员责任。

教师凡违反规定的,第一次发现,给予批评教育,并及时如数退还有偿补课所得。第二次发生的,不但要及时如数退还有偿补课所得,当事人该年度考核还要确定为不合格,不能作为各种评先评优的推荐对象,对情节严重的,将限期调离工作岗位并给予相应的纪律处分,并依据《 教师法》给予行政处分或解聘。

要通过对严禁教师有偿补课的专项治理工作,进一步规范办学行为,使教师有偿补课的现象得到基本遏制,创建行业新风,塑造教师良好形象,建设一支高素质的教师队伍,为深化教育改革,推进素质教育,促进学校科学发展、和谐发展、可持续发展创造良好的条件。

本系列工作方案为广州市汉达社会工作服务中心评估体系内容之一,如需使用,请征询本中心同意。

根据广州市天河区民政局相关文件要求,广州市汉达社会工作服务中心(以下简称“汉达”)对广州市天河区街道家庭综合服务中心开展xx-xx年度过程、中期和末期评估,现制定如下方案:

过程评估:随机不预约,共四次/家;

中期评估时间:根据家综项目周期而定,项目中期前后1个月左右;

末期评估时间:根据家综项目周期而定,项目末期前后1个月左右;

中心基本设置、运营管理能力、中心项目运作状况、项目成效、经费使用以及财务管理状况

评估采用街道家庭综合服务中心撰写自评报告、评估小组项目实地考察、听取介绍、查阅资料、社工面谈、利益相关者面谈、专家小组分析评定、现场反馈的方式进行。

由于家综初步建立,需要时间累积项目成效,因此短期内的评估,利益相关者由中心推荐,成效利益最大化归于家综。后期评估则会同时考虑在家综服务名单中随机抽取利益相关者。

各中心开展服务情况自检,撰写自评报告,并在约定时间内向评估小组公布。汉达负责组成评估组,执行现场考评、意见收集、效果评估、现场反馈等工作程序任务。

(六) 扫描过程评估文件,发送民政局

(一) 中心自评:评估前10个工作日,中心承办机构开展自评,并提交自评报告。

(二) 审阅自评报告:中心评估前5个工作日,评估小组审阅自评报告。

(三) 现场评估:评估小组前往中心进行中期评估。

(四) 撰写评估报告,中心评估后20个工作日内,汉达根据评估小组的评估反馈撰写评估报告。

(五) 评估报告反馈:完成报告后5个工作日内,评估小组和承办机构发送报告至中心,双方就评估报告内容进行反馈与沟通,达成共识。

(六) 递交评估报告:中心评估结束后25个工作日内,汉达将评估报告上报民政局。

(七) 领取评估报告:中心评估结束后,根据民政局要求领取报告。

(一) 参观中心场地。由中心负责人指引介绍中心场地,带领全体评委参观。

(二) 听取承办机构介绍项目成效。评估小组全体成员,街道分管领导、机构总干事或驻中心主任、及一线社工参加。

(三) 评委自由提问。

(四) 评委分组评估。中心各服务领域需至少安排一名社工参加,并准备好各领域文书档案,以备检查;同时需要中心协助安排2-3个房间。

(五) 评委分组评估。

(六) 评估小组集中,简要沟通反馈,交换评估意见。

(七) 评估小组给中心进行现场反馈,当日现场评估结束。

(一) 考核评估分值比例

考核评估结果采用百分制,以及采用节点式评估。中心年度总评分等于中期评估总分40%及末期评估总分60%相加所得。其中:

中期评估总分等于过程评估10%及中期现场评估90%相加所得。

末期评估总分等于过程评估10%及末期现场评估90%相加所得。

街道家庭综合服务中心的中心基本设置、运营管理能力、家庭服务中心项目运作状况、项目成效、经费使用以及财务管理状况共五个部分,过程评估、中期评估与末期评估分值比重有所偏重。

评估内容比重的调整,表明评估在不同阶段着重的内容不同,而总分节点所占比重的不同则意味着评估看重中心的成长能力。

(二) 评估等次

评估采用等级评定的形式。评定等级按优秀(90分以上)、良好(80-89分)、合格(70-79分)、基本合格(60-69分)、不合格(60分以下)5个等级予以评定。

为贯彻落实市政府《深入开展“安全生产年”活动实施方案的通知》(石政办函〔20xx〕38号)、市安委会转发xx省安委会《关于立即开展全国安全生产大检查的通知》(冀安委传〔20xx〕3号)和省教育厅4月16日电视电话会议精神,市教育局决定开展学校安全工作大检查,特制定实施方案如下:

以科学发展观为指导,坚持“安全第一、预防为主”的方针,落实上级有关指示、要求,按照“三进二十六防”涉及的内容,以排查、整改隐患为重点,确保我市各级各类学校安全。

此次检查从4月29日开始,到6月5日结束。采取各学校自查、县(市)、区督查、市教育局组织联查相结合的方式进行。

各学校自查时间:4月29日-5月10日,在认真自查的基础上填写《学校安全工作隐患排查表》,由学校校长、主管校长签字,上报县(市)、区教育局;

县(市)、区督查时间:5月11日-20日;

市联合检查时间:5月24日-6月5日,抽调各县(市)、区教育局主管局长、直属学校主管校长或安全科(处)长、主任,组成16个检查组,由市教育局统一安排,检查分工情况见附表三。

市教育局督查情况由各检查组长于6月20日前统一上报市教育局安全处;在学校自查和各县(市)、区督查期间,市教育局组将随时对各学校进行抽查,对检查不认真,排查不到位,整改不及时的要予以通报批评。

1.消防栓、灭火器等消防器材是否按标准配备,是否能正常使用;

2.消防通道是否通畅;

3.电线、线路是否存在老化、破损等情况;

4.消防疏散标志是否按要求进行设置。

门卫人员的配备要符合规定、值班登记情况、师生进出要有明显标志,坚决杜绝非本校车辆进入,未经允许无关人员不得进入校园和在校园门口逗留,学校是否按要求划定家长等候区。

2. 落实食堂值班,特别是夜间值班;

3. 坚持进料索证制度,把好来料进口关;

4. 食品卫生,做好食品留样,以备责任倒查。

2.校车的管理与使用是否符合安全规定;

3.体育器材设施是否安全适用;

4.实验室危险化学物品保管情况;

5.各类电器设备是否安全使用;

6.学校教室、实验室、礼堂、食堂、宿舍、厕所、游泳池等重要场所。尤其是可能受到滑坡、塌方、泥石流、河水危害位置的校舍,要采取切实有效的防范措施。

1.学校周边治安秩序问题;

2.学校门***通安全问题;

3.校园周边饮食安全问题;

4.校园周边文化环境。

各学校要按规定和时限立即行动起来,迅速组织开展自查自纠工作,各校校长要切实负起第一位的责任。各检查组对所抽查的每一所学校要认真排查,掌握情况,摸清底数,及时发现安全隐患,并认真填写“三进二十六防”配套的检查表。通过各种形式的检查和整改,使学校的安全工作更加抓实。

各学校对自查中发现的安全隐患和工作中的薄弱环节,要边检查边整改,以检查促整改。凡学校能够自己整改的要限期完成;学校整改有困难的,要逐级上报,限期解决。各县(市)、区教育局要及时检查和督促整改落实情况,整改要提出时间表并明确相关责任人。

此次检查是落实国务院、省、市安委会和市政府“安全生产年”活动的具体行动,检查工作一定要严肃认真,绝不能应付了事。对检查工作搞形式,走过场,整改不到位的责任人,将依据有关规定严肃追究有关人员的责任。

(一)食品卫生监督

1、加强餐饮消费环节的卫生监管。以餐饮单位、学校食堂、建设工地食堂、小饭桌为重点,巩固食品安全专项整治成果,落实《餐饮业和集体用餐配送单位卫生规范》及《餐饮业食品索证管理规定》,建立餐饮单位原料进货溯源、采购索证、进货验收台帐制度,严格食品索证索票管理,提高全区餐饮业食品卫生管理水平。

2、贯彻实施《食品卫生监督量化分级管理制度》。在食品生产经营单位继续实施卫生监督量化分级管理制度,严格掌握标准,全面量化,科学分级。指导企业对照食品生产、经营单位卫生监督量化指标的内容逐项落实。按照我市有关行业卫生标准,进一步规范我区小餐饮单位、前店后厂等食品行业的经营行为。

3、推进社区、乡村卫生监督网络建设。加强社区、农村食品安全、“农家乐”、旅游点等消费环节的卫生监管,推进社区、乡村卫生监督网络建设,保证广大消费者的饮食安全。

(二)医疗市场监督

1、开展医疗机构执法检查,规范其执业行为。按照省、市卫生监督部门的要求,深入开展专项整治,打击非法行医。重点查处无证行医,超范围执业、聘用非卫生专业技术人员、出租承包科室等违法行为。

2、加强采供血及临床用血机构监管,保障血液安全。加大对辖区内的单采供血机构、临床用血医疗机构的监督力度,建立医疗机构临床用血情况监督检查制度,进一步规范其执业行为,坚决杜绝违法违规采血浆行为,确保血浆质量及血液安全。

(三)职业卫生监督

1、做好职业健康监护,保护劳动者身体健康。摸清我区用人单位职业病危害的基本情况和从业人员的底数,突出对职业病危害的重点因素尤其是接触高毒物品和粉尘、劳动密集型企业等重点行业、重点人群的监督管理。集中开展中小企业危险化学品职业病危害专项整治,以存在有机溶剂、硫化氢、一氧化碳等易造成职业中毒的中小企业为重点,对我区化工、建材、冶金、塑料制造、印刷、等行业进行监督检查,切实保护劳动者身体健康。

2、落实放射工作人员证管理,开展放射卫生专项检查。做好《放射工作人员职业健康监护管理办法》(卫生部55号令)宣贯工作,按照规定要求,分别开展工业探伤和放射性同位素使用单位放射工作人员健康监护专项检查,重点检查放射诊疗许可,放射诊疗设备控制,放射工作人员培训、健康监护、个人剂量监测以及受检者防护等情况。

3、开展建设项目职业卫生审查。开展可能存在职业病危害因素的建设项目、放射性工作场所建设项目的职业卫生审查。突出新、改、扩建设项目职业卫生评价的评审;开展建设项目职业病危害评价情况监督检查,重点检查存在职业病危害的各类建设项目,对违法建设单位进行查处。

(四)传染病防治监督

1、加强传染病防治监督检查。以医疗废物处置、内窥镜和口腔器材消毒等为重点,加强传染病防治监督检查。重点检查医疗机构、疾病预防控制机构、采供血机构的传染病报告、预防控制措施、消毒隔离制度执行情况和医疗废物处置情况。

2、对餐具消毒等消毒服务机构进行专项整治。按照省、市有关卫生要求,开展以餐具消毒为重点的消毒服务机构专项整治,重点检查消毒服务机构消毒方法、工艺流程、设备、消毒效果、从业人员查体和培训、卫生条件等内容,加强餐具消毒服务需求单位采购、使用消毒物品索证检查,规范消毒灭菌服务,确保消毒物品卫生质量。
