1.1 甲方将对乙方拥有的叉车提供每季度的定期上门保养服务。每台车每年(或每_________小时)保养4次。

为了规范车间、厂区叉车的使用，特制订《叉车使用管理规定篇》。具体规定如下：

一、责任人

1.叉车司机是叉车驾驶、维护、保养，并为公司提供叉车服务的第一责任人。

2.叉车使用必须经过培训，叉车驾驶必须指定专人担任。未经叉车管理人员同意，除叉车司机以外的任何人不得驾驶叉车。

3.叉车行驶中驾驶员应集中注意力，不得饮食，打手机和携带他人等。

二、叉车驾驶应注意的问题

1.叉车启动异常应立即通知叉车管理部门进行维修，严禁擅自拆修叉车。

2.车辆启动前，必须检查叉车的制动，货叉，轮胎等，使之处于完好状态。叉车启动时应保持较低的启动速度，驾驶员要查看周围有无人员和障碍物，并注意避让。
